Planning and Paying for Nursing Care in Nottinghamshire
Nursing care sits at the more expensive end of residential care, and the reason is straightforward, since a registered nurse on duty around the clock is costly to provide. That cost is met from several directions at once, which is why families find this part confusing. A contribution towards the nursing element may come from the health service, the council may contribute towards the rest depending on means, and the balance falls to the person or the family.
Getting a care needs assessment
The assessment is free, is not means tested and records what a person can no longer manage safely, including whether their needs require a registered nurse rather than care staff. Ask Nottinghamshire County Council adult social care to carry one out, or ask the ward team to arrange it where the question has arisen during a hospital stay. A relative who has been providing care can request a separate assessment of their own needs. Keep the written outcome, because homes, funders and health teams all work from it.
Fees, contributions and health funding
Weekly fees vary between homes according to the room, the level of nursing involved and what sits outside the basic charge. Where a home is registered for nursing, part of the fee relating to the nursing element may be met by the health service rather than by the council or the family. A financial assessment of income, savings and usually property then sets what the person contributes towards the rest. Ask each home for a written breakdown showing what the fee covers, what is billed separately and how notice of an increase is given.
Choosing a Nursing Home in Nottinghamshire and What to Check
Reading the ratings
Of the eighty eight homes listed, seven are rated Outstanding and sixty two are rated Good, ten are rated Requires Improvement and nine are not yet rated. Look at the date of the inspection alongside the judgement, because homes change hands and change managers. Where a home has been asked to improve, read what inspectors actually found, since a concern about record keeping is a different matter from a concern about how residents were treated.
Questions specific to nursing care
Ask how many registered nurses are on duty during the day and overnight, and how often nursing shifts are filled by agency staff. Ask how the home manages pressure care, feeding difficulties, catheters and end of life care, and how it works with visiting health professionals. Find out who reviews medication and how quickly a change in condition reaches a doctor. Ask as well what the home does when a resident needs to go into hospital and whether the place is held open.
What a visit should tell you
Visit when the home is busy rather than at its quietest, and go more than once if time allows. Notice whether residents who cannot easily speak for themselves are included in what is going on around them. Watch how staff move between rooms and whether call bells are answered without fuss. A home that is comfortable being seen at an awkward hour is usually a home worth considering.
