Arranging and funding a care home place in the East

The assessment that shapes everything after it

Almost every route into residential care runs through a care needs assessment, and it is worth asking for one before homes are viewed rather than after. Anyone who appears to need care and support is entitled to an assessment whatever their income, and a relative can request it on their behalf. A practitioner from the local authority will look at mobility, memory, medication, continence, nutrition and safety, and will set out whether needs can still be met at home or whether a residential placement is the realistic answer. The written outcome is useful whoever ends up paying, because it tells a prospective home exactly what it would be taking on. Where the question arises during a hospital stay, the ward discharge team normally starts the process for you.

Working out who pays

Residential fees are charged weekly and cover accommodation, meals and personal care, though hairdressing, chiropody, newspapers and outings are usually billed separately. A financial assessment applies the national means test, and anyone holding capital above the upper threshold funds the placement themselves, at least until their capital falls towards the lower limit. The value of a property may be disregarded where a spouse or certain other relatives still live in it, and a deferred payment agreement can sometimes bridge the gap while a house is being sold. Where a resident has a clinical need, the funding picture changes, so it is worth asking whether NHS continuing healthcare has been considered. Ask any home for a written schedule of fees setting out what is included and what is charged on top.

Choosing between the care homes in the East

Read the report, not only the rating

A rating summarises one inspection carried out on a particular set of days, and homes on this page hold ratings at every point of the scale, including some carrying no rating yet because they registered too recently to have been inspected. Two homes described by the same word can be very different places. Open the report and look at which key questions were marked down, whether the concerns were clinical, administrative or cultural, and how long ago the visit happened. A home rated Requires Improvement that has since changed manager may be on a very different footing from the one the report describes, and the manager should be able to tell you exactly what has changed.

What to notice on a visit

Arrange to visit at an inconvenient hour, such as mid afternoon or during a meal, and stay longer than feels polite. Watch how staff speak to residents who cannot easily reply, and whether people are dressed, occupied and out of their rooms rather than sitting in a row facing a television. Ask how many staff are on duty overnight, what turnover has been over the past year, and how the home covers sickness. Ask what happens if needs increase, and whether that would mean moving again.

Questions worth asking before committing

Ask how care plans are written and how often families are involved in reviewing them. Ask about visiting arrangements, meals taken outside set times, and how the home handles a resident who becomes distressed at night. Ask what notice is required on either side, and read the contract before signing it. If the answers come readily and you are invited to look around freely, that says a good deal in itself.
