What home care in Wells involves and what to look for
Home care covers everything from a single short visit each morning to several calls a day, and occasionally live in support. Because the work happens in someone's own house, continuity and reliability matter more than almost anything else, and they are the hardest things to judge from a brochure.
Deciding what support is actually needed
Begin with the practical detail: what time of day help is needed, how long each visit should last, and what it has to cover. Personal care, help with medication, meal preparation, shopping and companionship are all common, but agencies vary in how they handle memory loss, moving and handling, or someone who is reluctant to accept help at first. Ask how many different carers would normally attend, since a rotating cast unsettles most people. Ask also what happens when a regular carer is on leave or off sick.
Judging an agency on its inspection record
Each rating rests on five judgements, covering whether a service is safe, effective, caring, responsive and well led. One agency on this list has been judged Outstanding and eleven are rated Good, while eleven have not yet been rated at all, which is common for newer registrations and not in itself a warning sign. Where a service sits at Requires Improvement or Inadequate, read what inspectors actually found, then ask the manager directly what has changed since the visit. Check the inspection date as well, because a service can look quite different a year or two on.
Getting an assessment and sorting out the funding
Care at home is charged by the hour or by the visit, so the sums are easier to follow than residential fees, though the funding route is much the same.
Asking for a care needs assessment
Anyone who appears to need care and support can request an assessment, whatever their savings, and there is no charge for it. It looks at personal care, mobility, nutrition, memory, medication and the risks of managing alone, and ends in a written record of the support required. Somerset Council Adult Social Care carries out that assessment and the financial assessment that decides what public funding contributes. Where funding is agreed, ask whether it would come as a direct payment, since that usually gives a family more say over which agency is used.
What the hourly rate covers
Agencies set their own rates, which reflect the time of day, the length of each visit, the skills required and whether weekends or bank holidays are involved. Ask for the rate in writing, ask what the minimum visit length is and ask how travel time between calls is treated. Check how changes are handled if needs increase or decrease, and what notice either side has to give. Ask too what the agency does if a carer cannot reach the house, because that answer tells you a great deal about how it is run. Where the council is contributing, check whether its rate covers the full hourly charge or whether a difference would be left for the family to meet.
