How do we request a care needs assessment in Cornwall?

Contact the adult social care team at Cornwall Council and explain what has changed for the person clinically and practically. The assessment is free and available to anyone who appears to need care and support, whatever their income or capital. It will establish whether nursing care is genuinely required or whether a residential home would meet the need, which affects both the fee and the range of homes worth visiting. Ask for a written copy before approaching homes.

Can Cornwall Council pay for a nursing home placement?

It may contribute, and the financial assessment decides how much. Cornwall Council weighs income and capital alongside the care needs assessment when settling what it will pay. Separately, the health service may contribute towards the nursing part of the fee where care from a registered nurse is needed, and continuing healthcare may cover the full package where health needs are substantial. Ask whether assessments for either have been carried out, because they are decided outside the council process.

What is the difference between a nursing home and a residential care home?

A residential care home provides accommodation, meals and help with personal care, staffed by care workers rather than nurses. A nursing home offers the same and additionally has registered nurses on site around the clock. That matters where someone needs wound dressings, a feeding tube, catheter care, monitoring of an unstable condition or a complex medication regime. The category is fixed at registration with the Care Quality Commission and can be checked before you visit.

How far ahead should we start looking for a nursing home?

As early as circumstances allow, because nursing beds are scarcer than residential ones and assessments take time to complete. Arranging a care needs assessment can run to several weeks, and a home experienced in a particular condition may have no vacancy at all. Starting early lets you see two or three homes properly rather than accepting the first offer. Where a hospital discharge is setting the timetable, make clear to the ward early that you want a say in the decision.

What should we ask when visiting a nursing home in Cornwall?

The care needs assessment

A care needs assessment is free and open to anyone who appears to need care and support, whatever their savings. Cornwall Council is responsible for carrying it out. As well as recording what help is required with washing, dressing, medication, mobility and overnight support, it establishes whether a nursing home is necessary or whether a residential home would meet the need. Families sometimes find the answer is not the one they assumed, and since the two categories differ in cost and availability, having that recorded in writing is genuinely useful.

Three possible sources of funding

The council's financial assessment measures income and capital against the national threshold and sets its contribution, with property treated according to who else lives there. Where someone is assessed as needing care from a registered nurse, the health service may pay towards the nursing element of the fee. Where health needs are substantial and ongoing, continuing healthcare may cover the whole package instead. The latter two are health service decisions rather than council ones, so ask directly whether either assessment has been completed.

What to establish about the weekly fee

Ask in writing what the figure covers and which items sit outside it, such as hairdressing, chiropody, escorted appointments and outings. Ask how often fees are reviewed and what the recent pattern of increases has been. Where a third party top up above the council rate is proposed, get the arrangement and its expected rises documented before agreeing to anything, and ask what would happen if savings fell to the threshold during a stay.

Choosing the right nursing home in Cornwall

Reading past a common grade

Thirty six homes holding a Good rating means the published evidence is encouraging but not discriminating. The reports are where the differences appear, and the sections on safety and effectiveness are the ones that reflect nursing quality most directly. For the six homes at Requires Improvement, read what inspectors actually found before ruling anything out, since governance findings and care findings are not the same thing. Two homes carry no rating, usually after a recent registration, and there a thorough visit has to substitute for a report.

The questions that reveal clinical quality

Ask how many registered nurses are on duty during the day and overnight, and how a shift is covered when one is absent. Ask what proportion of nursing shifts is filled by agency staff, because a settled team recognises deterioration sooner. Ask which conditions the home supports routinely, whether that is advanced dementia, pressure wounds, a feeding tube or end of life care, and how it works with visiting general practitioners and community teams. Ask what happens when someone becomes acutely unwell at night, and whether rising needs would mean a move elsewhere.
