Choosing Between the Nursing Homes in Exeter
Fifteen homes is a field worth going through carefully, and the published ratings are unusually strong here, which makes the choice harder rather than easier. Three homes are rated Outstanding, ten are rated Good and one is rated Requires Improvement, with one not yet inspected, so most of the shortlist will come down to differences the ratings cannot express.
What the Ratings Do and Do Not Tell You
Every rating is assembled from five key questions, asking whether a service is safe, effective, caring, responsive and well led. A home rated Good overall can carry a weaker judgement on one of those questions, and a home rated Outstanding earned that on the day it was inspected rather than permanently. Check the date alongside the outcome, since a judgement made some time ago may predate a change of manager, a change of owner or a completed improvement plan. Where a home has not been inspected yet, there is simply no independent view of it, which calls for more thorough questions rather than suspicion.
What Nursing Care Actually Provides
The defining feature of a nursing home is that registered nurses are on duty as well as care staff, so clinical needs are met on the premises rather than through visiting professionals. That matters where someone needs wound care, complex or injected medication, support with a catheter or feeding tube, or skilled care as they approach the end of life. It is worth asking each home which of those needs it manages routinely, because registration for nursing does not mean every home takes on every condition.
Questions Worth Asking on a Visit
Ask how many registered nurses are on duty across the day and overnight, how sickness and leave are covered, and how much the home relies on agency staff. Ask how long the manager has been in post and what would prompt a resident to be moved elsewhere. Watch how the afternoon is spent and whether staff address residents by name. Two visits, one at a quiet hour and one at a mealtime, are more revealing than any tour arranged in advance.
Arranging and Funding Nursing Care in Exeter
Alongside the visits sit two assessments, one establishing the level of care needed and the other deciding who pays for it. Both take time, so starting them while the shortlist is still forming is the sensible order.
Requesting a Care Needs Assessment
A needs assessment is free and open to anyone who appears to need care and support, whatever their savings. Devon County Council Adult Social Care is the authority responsible for carrying it out for people living in Exeter and for arranging any funded support that follows. A practitioner will go through mobility, washing and dressing, medication, continence, memory and overnight needs, and will consider how much of the current arrangement depends on a relative. Those relatives can request a carer's assessment of their own, which is a separate entitlement.
Fees and the Contributions Available
A financial assessment considers income, savings and, for a residential placement, usually the value of a property, measured against the national capital thresholds. Above the upper threshold a family funds the placement itself, and below it the council contributes on the basis of the means test, with a top up sometimes needed where a chosen home charges more than it will pay. Nursing home residents assessed as needing care from a registered nurse also attract NHS funded nursing care, paid directly to the home towards that element of the fee. Ask each home for a written fee schedule and check what sits outside it.
