About Community Reablement Team
We are a community reablement team working across Middlesbrough and the Redcar and Cleveland area, supporting older people and younger adults in their own homes. Reablement is a particular way of working. Rather than taking tasks over, we support people to regain the skills and confidence they need to manage for themselves again, often after illness, a fall or a spell in hospital. That means resisting the temptation to do things quickly on somebody's behalf, and instead giving them the time, the encouragement and the practical help to do it themselves. It can be slower in the short term and it is usually far better in the long run. We start by finding out what a person could do before, what they can manage now, and what they most want to get back to. Sometimes that is washing and dressing without help. Sometimes it is cooking a meal, getting up the stairs, or being able to get out of the house alone again. We agree realistic goals and we adjust them as progress happens. We work closely with therapists, general practitioners, hospital teams and social workers, and we keep families informed about how things are actually going.
