About Total Community Care

Total Community Care supports people in their own homes across Telford and the wider Telford and Wrekin area. We work with older people and with younger adults, and the needs we cover range from a little practical help each morning to full packages built around complex and long standing conditions. We support people living with dementia, adults with learning disabilities and autistic adults, people managing mental health conditions, people with physical disabilities and people with sight or hearing loss. What holds all of that together is the way we plan. We start by asking what the person wants their day to look like, what they can do themselves and where help is genuinely useful, and we write the plan around those answers. Consistency matters. We try to keep the same small team visiting the same people, we brief carers thoroughly, and we keep families in the picture without taking decisions away from the person we support. Our carers are trained for the particular needs of those they visit, including communication, moving and handling, medication and, where relevant, positive behaviour support. Training is not a single event here, and neither is supervision. The Care Quality Commission rated us good at our most recent inspection.
