We build each plan of care with the person it belongs to. What time they want us, what they want help with, what they would rather keep doing themselves, and what they would like us to leave well alone. That plan gets reviewed as things change, not filed away. Wherever we can, we send the same carers to the same people. Familiarity is not a luxury in this work.
