We begin by asking what help is actually wanted rather than working from a form. People are usually very clear about what they can still manage and would rather keep doing for themselves, and respecting that is as much the job as the tasks we take on. With dementia, continuity is not a nice extra. A carer the person recognises, arriving at roughly the time they expect, achieves far more in half an hour than a stranger could in twice that.
