It is their home, their routine and their decision how help happens, and we fit around that rather than expecting the reverse. We visit first and take our time over the conversation, because what families assume is difficult and what the person finds difficult are often not the same thing. From that conversation we agree a plan, with family or an advocate involved whenever somebody wants them there, and we revise it as circumstances change rather than at fixed review dates. Visits cover personal care, medication, meals and drinks, housework and laundry, shopping, getting to appointments, and company for people whose days have grown quiet. For people living with dementia we keep the same small team visiting and work at their pace. Where somebody is managing a mental health condition we keep support consistent and work alongside their community team. Where there is a physical disability we look closely at equipment, transfers and how the home is arranged. We are strict about timekeeping and we would much rather hear early that something is not working than find out later.
