About Housing 21 - Meadowfields
We are the Housing 21 care team at Meadowfields in Thirsk, supporting people who live in their own flats here and in the surrounding area. We work with older people and younger adults, people living with dementia or a mental health condition, people with a learning disability or autism, people with a physical disability, people with a visual or hearing impairment, and people dealing with alcohol or drug misuse. What we offer is support that fits around somebody's own front door. People here have their own home, their own routine and their own visitors, and the care comes to them rather than the other way round. Support covers personal care, help with medication, meals and drinks, and a hand with the practical parts of the day that have become harder. The amount of help needed rarely stays the same. Some people want very little and simply like knowing someone is nearby. Others need several calls a day. We review it openly rather than leaving an arrangement in place because it is on the schedule. Being on site means we notice things, which is often what keeps somebody living independently for longer. Our most recent inspection by the Care Quality Commission rated us good overall.
