About 360 Complex Care South West

360 Complex Care South West supports people with complex health and disability needs in their own homes across Devon, working out of Exeter. We work with autistic adults and adults with a learning disability, people with physical disabilities and those with sight or hearing loss. Complex care at home means clinical support delivered in somebody's own house rather than on a ward, and it only works if the team around a person is properly trained, properly supervised and properly consistent. We build a small dedicated team around each person, train them on that individual's specific needs rather than in the abstract, and keep the same faces going in, because in this work an unfamiliar carer is not just unsettling, it is unsafe. Beyond the clinical side, the point is that somebody gets to live their own life. So we plan support around what the person wants their week to contain, whether that is college, work, family, getting out or simply a routine they control, and we write it in a form they can actually use. We work closely with families, commissioners, community nursing teams and specialists, and we expect families to tell us bluntly when we have got something wrong. We are a newer team and have not yet had a full inspection of our own. Please ask us searching questions before deciding. Enquiries and referrals across the South West are welcome.
