Arranging nursing care in Richmond and meeting the cost
A nursing placement requires a clinical judgement as well as a social one, and that judgement shapes both the shortlist and the funding that follows.
The assessment and who carries it out
An assessment of care needs is free, is not means tested and is open to anyone who appears to need care. Where nursing is in question, a nurse assessor usually contributes, looking at wounds, continence, swallowing, medication and how often clinical attention is needed overnight. London Borough of Richmond upon Thames Adult Social Care carries out that assessment and the financial assessment that follows. Where nursing needs are substantial, the assessment should also consider whether continuing healthcare funding applies, which is decided separately from the council's means test.
Fees and the nursing contribution
The financial assessment weighs income, savings and capital. Above the national threshold a placement is privately funded; below it, the council contributes up to a level it considers sufficient for assessed needs, and a third party top up may be sought where a more expensive home is preferred. Nursing homes attract a separate contribution towards the cost of registered nursing care, paid by the health service directly to the home. Ask each home for its weekly rate in writing and for confirmation of how that nursing contribution appears on an invoice.
Choosing between three nursing homes
A narrow list makes each visit count, and it makes looking slightly beyond the area worth considering.
What the ratings tell you
All three homes here are rated Good, so the grade will not separate them. Read instead what inspectors said about nurse staffing levels, medicines management, pressure area care and how well clinical records are kept, since those are the areas where nursing homes differ most. Note the date of each inspection, because nurse recruitment can change a home quickly in either direction. Where a report is some years old, treat it as a starting point and ask the manager what has changed since.
What to ask on the visit
Ask how many registered nurses are on duty by day and overnight and how the home covers a nurse who calls in sick. Ask how it works with local GPs and district nursing, how quickly it responds to a resident who is deteriorating, and how it supports families at the end of life. Watch whether call bells are answered promptly and whether people are supported patiently at mealtimes. Ask finally what the home changed after its last inspection and compare that answer with the published report.
