About Council Reablement Service (Hub)
We are the reablement team based in Trowbridge, run by Wiltshire Council, working with adults in their own homes across the area. Reablement is short term by design. We come in after an illness, a fall, a spell in hospital or a change in somebody's circumstances, and the aim is to help them get back to doing as much for themselves as they can rather than to take things over. That means we sometimes stand and wait while somebody manages a task slowly, because that is how confidence and ability come back. We work with older people and people living with dementia, adults with learning disabilities or autism, people living with mental health conditions, people with physical disabilities, people with a visual or hearing impairment, and younger adults. Depending on the person, support might cover personal care, medication, meals, getting about safely in the house, and practical help with equipment. At the end of a period of reablement, some people need no further support and others need a longer arrangement put in place, and we are honest with people about which it looks like.
