About Council Reablement Service (Hub)
We are the Wiltshire Council reablement team based at the Chippenham hub, supporting people in their own homes across the county. The people we work with include older people, adults living with dementia, autistic adults and people with a learning disability, people managing mental ill health, adults with physical disabilities, people with sight or hearing loss and younger adults. Reablement is short term support with a clear purpose. Most people come to us after a stay in hospital, a fall or an illness that has knocked their confidence and their capability. Our job is to help them get back to doing as much as they can for themselves, in their own home, rather than settling into a long term care package by default. That means we work differently from ordinary home care. We stand alongside somebody while they make their own breakfast rather than making it for them, we practise the stairs, the shower and the kitchen, and we bring in equipment and small adaptations where those make the difference. We work closely with occupational therapists, physiotherapists, GPs and hospital teams throughout. Not everybody regains everything, and where ongoing support is needed we help arrange it honestly rather than pretending otherwise. Our most recent inspection by the Care Quality Commission rated us good overall. If you or a relative have been referred to us, please ask us anything about how the period will work.
