Others need several longer calls a day, and needs shift over time, so we review arrangements honestly rather than letting a plan sit unchanged. Two things take up most of our attention. One is arriving when we said we would, because a visit at an unpredictable hour causes more anxiety than it relieves. The other is continuity, because a carer who already knows somebody is the one who will spot that they are eating less or seeming muddled this week, and that noticing is often what keeps people out of hospital.
