About Outreach
We are an outreach team supporting people in their own homes across Waltham Forest. Our work is about helping people stay connected to their own lives, in their own communities, with support arranged around them rather than around a building. We support older people and younger adults, people with a learning disability, autistic people, people managing mental health conditions, people with physical disabilities and people living with sight or hearing loss. What that looks like varies completely from one person to the next. It might be personal care and help with medication. It might be support to get to college, work or an appointment, to manage a household, to handle money, or to do the things that have become daunting to face alone. We start every arrangement by asking what the person wants their week to look like, and we build support around the answer. The plan is written down clearly and revised when things change. Where somebody needs routine and advance warning of anything different, we build that in from the start. Continuity is something we protect. Support only works when the person and the worker actually know each other, so we take care over matching and we listen when it is not right.
