About Reablement Service Waterton Hub
We are the reablement team based at the Waterton Hub in Wakefield, working with people in their own homes across the district. Reablement is short term support with a definite purpose. Somebody may have come out of hospital, had a fall, or found over a hard few months that washing, dressing, cooking and getting about have slipped out of reach. We come in for a period of weeks and work alongside the person to rebuild what they can manage themselves, rather than taking those tasks over for good. A great many people finish with us needing much less help than when they started. We support older people and people living with dementia, adults with a learning disability and autistic adults, people managing a mental health condition, people with a physical disability or a visual or hearing impairment, younger adults, and people whose lives have been affected by alcohol or drug use. Each person has goals agreed with them at the start, in their own words, and we measure progress against those rather than against a general idea of improvement. Where longer term support turns out to be needed, we help put it in place before we step away. We work closely with hospital discharge teams, therapists, GPs and social workers.
