About Hazel Garth Reablement Hub
Hazel Garth is a reablement hub based in Knottingley, working with people across the Wakefield district in their own homes and community settings. Reablement is short term and purposeful. Our aim is not to take tasks over but to help someone get back to doing them, usually after a hospital stay, an illness, a fall or a period when things have become unmanageable. That means the support we provide should reduce over time rather than grow. We work with older people and people living with dementia, with adults living with mental health needs and with people recovering from problems with alcohol or drugs. We support adults with a learning disability or autism, people living with a physical disability and people with a visual or hearing impairment, and we work with younger adults as well as older ones. In practice, our staff spend time doing things alongside someone rather than for them, whether that is getting washed and dressed, preparing a meal, managing medication or building the confidence to leave the house again. We look at equipment, at the layout of a home and at what small changes would make independence possible. We are honest when reablement is not the right answer and someone needs ongoing support instead, and we say so rather than stretching things out.
