We agree each package with the person and the people close to them, and we write down clearly what every visit is meant to achieve so nobody is left guessing. Keeping the same carers with the same households is something we plan for rather than hope for, and where a change cannot be avoided we say so in advance. We review support as circumstances change instead of letting a plan go stale, and we would far rather hear about a problem early than be told about it months later.
