What people need at that point is practical help and encouragement to get back to doing things themselves, not a service that quietly takes over and leaves them less able than before. So we work towards people needing us less wherever that is realistic. Everything begins with a visit and an unhurried conversation about what has genuinely become difficult. From that we agree a plan with the person, involving family or an advocate if they want that, and we review it often, because at this stage things really do change from week to week. Visits cover personal care, medication, meals and drinks, help around the house, shopping, getting to appointments and company. Where somebody is not going to regain what they have lost we say so honestly and help them plan for longer term support.
