About Community Crisis Support Service
We are a crisis support team providing care in people's own homes across Nottingham and Nottinghamshire, working with older people, younger adults and people living with dementia. Our work usually begins at short notice. Somebody has come out of hospital sooner than expected, a family carer has been taken ill, a fall has changed what is possible overnight. At that point people need help quickly, and they need it from somebody who will look at the whole situation rather than deliver a fixed package. So we move fast and we keep asking. What has actually changed, what does the person want, what will make the next fortnight workable. Plans are agreed with them, and with family or an advocate where they want that, and reviewed frequently because at this stage circumstances shift from day to day. Visits can cover personal care, medication, meals and drinks, help getting about safely, housework, shopping and company. Where somebody is living with dementia we keep the visiting team as small as the situation allows, since new faces in a crisis are the last thing that helps. We work closely with hospital teams, GPs, district nurses, therapists and social workers, and we are honest when a short term arrangement is not going to be enough. Our aim is usually to hand over to something settled rather than to stay indefinitely. Our most recent overall rating from the Care Quality Commission is good. Please get in touch if you need to talk something through quickly.
