Everything agreed goes into plain writing and is revised when circumstances change rather than left in place because it suited months ago. Where dementia or mental ill health is part of the picture, our carers are expected to slow down, explain clearly and allow time for somebody to take in what is happening. Rushing is usually what turns a manageable morning into a distressing one.
