About Reablement at Home

We provide reablement support at home in Grimsby and the surrounding area. Our purpose is to help people get back to managing for themselves, usually after a spell in hospital, a fall, an illness or a period when things at home have become unmanageable. Where ordinary care does something for a person, we try wherever possible to help them do it again. That means working on the things that actually matter at home: getting out of bed and moving about safely, washing and dressing, preparing food and drink, managing medication and getting back out of the front door. Progress is often slow and rarely in a straight line, and the gains that count are frequently small ones. We work with older people and younger adults, including people living with dementia, adults with learning disabilities, autistic people, people with physical disabilities, people with visual or hearing impairments and people living with eating disorders. Where it becomes clear that longer term support is what somebody needs, we say so plainly rather than carry on. Our staff are trained for the needs they are asked to meet and work alongside therapists, nurses and general practitioners. At our most recent inspection the Care Quality Commission rated us outstanding overall. We are pleased with that and conscious that a rating describes a moment rather than a guarantee.
