About London Borough of - Reablement Service
We are the reablement team for the London Borough of Merton, based in Morden and working with residents in their own homes across the borough. Reablement is short term support with a clear purpose. Someone may have come out of hospital, had a fall, or simply found over a difficult few months that washing, dressing, cooking and getting about have become hard. We come in for a period of weeks and work alongside the person to rebuild what they can do for themselves, rather than taking those tasks over permanently. Many people finish with us needing far less help than when they started, and some need none at all. We support older residents, people living with dementia, adults with a learning disability or who are autistic, people managing a mental health condition, people with a physical disability, and younger adults. Each person has a plan agreed with them setting out what they want to get back to, and we judge how things are going against that rather than against a general notion of progress. We work closely with hospital discharge teams, therapists, social workers and GPs. Where longer term support turns out to be needed, we help arrange it before we step away, so that nobody is left without a plan. Our most recent CQC overall rating is good.
