About H2H Community Care
H2H Community Care supports people in their own homes in Lincoln and across Lincolnshire. We work with older people, younger adults, people living with dementia, people managing mental health conditions, people with physical disabilities and people with sight or hearing loss. Almost everyone who contacts us wants the same thing. To stay in their own home, with their own belongings and their own routine, and to have just enough help to make that workable. Our job is to provide that help without taking the household over. We begin with an unhurried visit and a conversation about what is genuinely difficult, which is not always what people expect. Sometimes it is washing and dressing. Sometimes it is medication, or meals, or getting to appointments, or simply that the days have grown very long. From that we agree a plan, with family or an advocate involved if the person wants, and we revisit it as things change. Consistency is where home care lives or dies, so we keep the team around each person small and introduce new staff properly rather than sending somebody unannounced. For people living with dementia and for people who cannot see well, a familiar voice at the door matters a great deal. We are clear about our limits. If somebody needs more than we can safely provide we will say so and help them find the right support. Our most recent overall rating from the Care Quality Commission is good.
