From that conversation we agree a plan, with family or an advocate involved whenever somebody wants them there, and we revise it as circumstances change rather than at fixed review dates. Visits cover personal care, medication, meals and drinks, housework and laundry, shopping, getting to appointments, and company for people whose days have grown quiet. For people living with dementia we keep the same small team visiting and work at their pace. Touch, music, familiar smells and an unhurried presence often reach somebody in advanced dementia when words no longer do, and we make room for that rather than treating a visit as a list of tasks. Where somebody is managing a mental health condition we keep support consistent and work alongside their community team.
