Our carers are local and our rounds are planned so that visits actually land when they are supposed to. If somebody needs help to get up, a late call is not a minor inconvenience, it is an hour of their morning lost. We begin with a visit and a conversation at home, with family involved if that is what the person wants. The plan we write is built around the routine that already exists rather than a new one we have decided on, and we review it as things change.
