We visit before anything begins, agree the plan with the person and anyone they want present, write it in plain words and review it as circumstances change. We say honestly when we think somebody needs more support than can safely be provided at home. Keeping the same carers going into the same households is something we work at, because familiarity is how changes get noticed early.
