Our first job is to sit down with the person, and whoever they want with them, and work out what has actually become difficult. From there we agree a plan: it might be a short call each morning for washing, dressing and medication, or several visits a day covering meals, shopping and company, or a few hours that give a family carer a proper break. Continuity is the thing we are asked about most often. We keep the group of carers visiting each household small and familiar, and we introduce anyone new properly rather than sending an unfamiliar face unannounced. Our carers are asked to notice as well as to do. If someone is eating less, seeming low or becoming unsteady, we want to know the same day so that we can speak to the family or the GP early.
