Visits are built around the day the person wants to have rather than the day that suits a rota. If somebody has always been up by seven, we do not ask them to start waiting until nine. Where we cannot do something, we say so plainly instead of promising and then disappointing. We keep the same small group of carers going to the same people. Familiarity is what makes personal care bearable for the person receiving it, and a regular carer notices the missed meals or the new unsteadiness long before anybody else.
