About South West Supported Living
South West Supported Living provides care and support to people in their own homes in Fareham and the surrounding part of Hampshire. The people we work with include older people, younger adults, people living with dementia, people with learning disabilities, autistic people, people managing mental health conditions, people with physical disabilities and people with sight or hearing loss. Supported living rests on a simple idea. The tenancy is the person's, the home is the person's, and support comes to them on terms they set. It is not a placement and we are not in charge. Our job is to make a particular life workable, which for many of the people we support means keeping a home, a job or a college place, seeing friends and managing money with the right amount of help and no more. Plans are written with the person, in a form they can actually use, involving family or an advocate if they want that, and revised as things change. We keep the team around each person small and introduce new support workers properly. For autistic people that consistency matters more than anything, along with notice of change well in advance and attention to sensory needs. Where somebody communicates through signing, pictures or objects, our staff learn their way of doing it. Support can cover personal care, medication, cooking and shopping, help with correspondence, getting out, and somebody alongside on a difficult day. Please get in touch if you would like to talk about support in the area.
