The plan is agreed together and changes as life changes rather than when a review date arrives. Where somebody has a physical disability we pay attention to the mechanics of the house, since whether a frame is within reach or help arrives at the right moment decides a great deal. Where somebody communicates in their own way we learn it and record it so the whole team responds alike. For people living with dementia, familiarity and timing do more good than anything else, so we keep teams small and say in advance when a new carer is coming. Our carers are checked, trained and supervised, and we are registered with and regulated by the Care Quality Commission.
