We review the arrangement when circumstances change rather than at arbitrary intervals. We keep the team around each person small and introduce new staff properly rather than sending somebody unannounced. Our office is contactable during the day and there is somebody on call outside those hours, because problems in home care rarely arrange themselves around working time. We are also clear about our limits. If somebody needs more than we can safely provide we will say so and help them find who can.
