We support adults who have a learning disability, autistic adults, people living with mental health conditions, people with a physical disability and younger adults needing support to live independently. We keep teams small around each person on purpose. It spares people from constantly meeting strangers, and it means our carers know somebody well enough to spot when they are not themselves and raise it early with family or a GP. Everything starts with a home visit and an honest conversation about what would genuinely help. The plan is agreed with the person and reviewed as circumstances change.
