About Collaborative Healthcare
Collaborative Healthcare provides care in people's own homes in Slough and the surrounding area, working with older people, adults living with dementia, adults with a learning disability, autistic adults, adults with mental health needs, adults with a physical disability, people with sight or hearing loss and younger adults. Most people who contact us want the same thing, which is to stay in their own home with just enough support to make that work. So we begin by asking how somebody actually lives rather than by offering a package. For one person that is a short call each morning to get the day started safely. For another it is help with medication, meals, shopping and laundry, support getting to appointments, or longer visits that give a family carer a proper rest. For a younger adult it is often the practical support that makes work, study and independence possible. We aim to send the same familiar carers to each person, we agree the plan with them and with the people who matter to them, and we review it when things change. If we notice something is wrong, whether a fall, a lost appetite, a missed dose or a mood that has shifted, we raise it promptly with the family and with health professionals as appropriate. The Care Quality Commission has not yet published an overall rating for us. In the meantime we are happy to be judged on the questions we answer, the plan we put in writing and what the people we already support have to say.
