About Home Independence Team
We are the Home Independence Team, based in Beaconsfield, and we provide care and support in people's own homes across this part of Buckinghamshire. We support older people, including many living with dementia, adults with learning disabilities and autistic people, people with physical disabilities and younger adults. Independence is the word in our name and it is meant seriously. Our aim is not to take tasks off people but to make it possible for them to keep running their own lives, in their own homes, on their own terms. That means help where help is genuinely needed and nowhere else, and it means listening to what somebody wants instead of deciding on their behalf. Every arrangement begins with a conversation in the person's own home. How does the day run now? What has become difficult? What do you still want to do for yourself, and where would a hand actually help? From those answers we agree the visits, and we revise them as needs move. Support might mean help with washing and dressing, a prompt and a check with medication, a meal cooked and eaten with company, help getting safely about, assistance with shopping and appointments, or support to get out to work, college and the community.
