We are Collective HealthCare, a home care agency supporting people in Aylesbury and the wider Buckinghamshire area. We provide care in people's own homes, arranged around their routines rather than around ours. Most of the people we visit are older adults who want to stay independent for as long as they can. We help with personal care, medication, meals, household tasks and getting out of the house, and we support people living with dementia, keeping days calm and familiar and taking the trouble to learn what matters to each person. We also work with younger adults and with people living with a physical disability, where the emphasis is often on keeping someone in control of their own routine rather than fitting them into somebody else's. Every arrangement starts with a proper conversation about what would genuinely help. From there we agree a plan in writing, and we review it as needs change. We aim to send the same carers to the same households, because familiarity makes care easier for everyone and means changes get spotted early. We do not yet hold an overall rating from the Care Quality Commission. Rather than ask anyone to take our word for it, we would encourage families to question us closely, meet the people who would be visiting, and hold us to what we say.
