About Forest Borough Council - Intermediate Care
We are the intermediate care team at Bracknell Forest Borough Council, supporting adults at home across the borough. Intermediate care is short term by design. Most people come to us at a turning point: after a spell in hospital, after a fall, or when things at home have become difficult enough that the next step might otherwise be a care home. Our job is to give intensive, practical support for a limited period so that people can regain what they have lost and stay where they are. That means personal care, support with medication, help with meals, and above all working on the things that have become hard again, whether that is getting up the stairs, managing in the kitchen or building confidence back after a fall. We work closely with therapists, nurses and social workers, because intermediate care only works when everybody is pulling the same way. We support older people, adults living with dementia, people with a learning disability, autistic adults, people living with mental health conditions, people with physical disabilities, people with a visual or hearing impairment and younger adults. The aim throughout is to hand independence back rather than to take it over, and to be honest with people and their families about what is realistic.
