Weighing Up the Nursing Homes in York
What the current ratings show
The inspection picture across the homes listed here is reassuring on the whole. Connaught Court holds an Outstanding rating, nine further homes are rated Good, among them Birchlands Care Home, Fulford Nursing Home, Meadowbeck, Mulberry Court and Somerset House Nursing Home, and three homes have no published rating yet. Chocolate Works Care Village, Goldsborough Manor Care Home and The Oaks fall into that last group. An absent rating usually reflects a service registered too recently to have been inspected, so ask the manager when a first inspection is anticipated.
Reading past the headline word
Each rating brings together five separate judgements, covering safety, effectiveness, care, responsiveness and leadership. Two homes can share the same overall word while differing sharply in the detail, so the sections on safety and on leadership are the ones worth reading closely. They tend to reveal how a home behaves when staffing is tight or when a resident deteriorates unexpectedly. Where a report is a year or two old, ask the manager what has changed since, and whether the team that earned the rating is still in place.
Matching a home to the person
The right choice depends as much on the individual as on the paperwork. Ask whether the staff have cared for someone with the same diagnosis, how nursing cover is maintained overnight and at weekends, and how the home communicates with families when something changes. Some homes here specialise in particular kinds of care, so be clear about what is needed before ruling anything in or out. Visiting during a busy part of the day will tell you more than any prospectus, particularly if you watch how staff speak with residents who cannot easily reply.
Arranging and Paying for Nursing Care in York
Beginning with the needs assessment
The proper first step is an assessment of need, and it is open to anyone who appears to require care and support. York City Council Adult Social Care carries out that assessment and decides what funded support should follow. It is free, it is not means tested, and a relative or advocate can request one on someone else's behalf. An assessor will look at mobility, medication, continence, nutrition, memory and how much supervision is needed at night, then produce a support plan setting out the kind of setting likely to meet those needs.
Working out who pays
A financial assessment then considers capital and income in order to calculate a weekly contribution. Where savings sit above the national upper capital limit the placement is normally self funded, and where they fall below it the authority contributes towards an agreed rate. Nursing placements may also attract the NHS funded nursing care payment, which covers the registered nursing element of the fee and is paid directly to the home. If the dominant need is a health need, it is worth asking for an assessment for NHS continuing healthcare, which can meet the entire cost.
Understanding what is being charged
Weekly fees differ between homes according to the room offered, the amount of nursing time required and how generously a home staffs its shifts. Providers set their own rates, so a published average is of little help when planning. Ask for the fee in writing for the particular room, along with a list of anything billed as an extra, the notice period and how often charges are reviewed. If council funding may be needed later, say so at the outset rather than once a contract is in place.
