About The Weir Nursing Home
The Weir is a nursing home in Hereford caring for older people and younger adults, including people living with dementia, people with physical disabilities and people with sight or hearing loss. Qualified nurses are on duty day and night. Having nursing on site means clinical care is given here as it is needed rather than pieced together around visits from elsewhere. Just as important, our nurses and carers work as one team, so what somebody enjoys, what unsettles them and how they want their day to run counts alongside the clinical record. We plan for each person individually. Some need nursing support delivered precisely and reliably. Others mainly want comfort, good food and help keeping in touch with the people who matter to them. Where dementia is part of the picture we keep the day familiar and unhurried, and we use what families tell us about a person's life rather than storing it away in a folder. Meals are cooked here and adapted where swallowing or appetite is difficult. Where sight or hearing loss is involved, we adjust how we communicate and make the building easier to navigate. Short stays are available. Families are welcome at any reasonable hour and we tell them honestly how somebody is doing. Our most recent rating from the Care Quality Commission is good. Please ring to arrange a visit.
