We agree the shape of the support with the person rather than presenting a fixed package, and we review it honestly when circumstances change. If somebody needs more from us we will say so, and if what we are providing has stopped matching the situation we would rather raise it than let it drift. For people living with sight or hearing loss, small practical things decide whether somebody stays independent, and our carers are asked to pay attention to them. Continuity is what we plan our rotas around. Personal care is much easier to accept from a familiar face, and a carer who knows somebody's usual self notices quickly when they are unwell.
