About Stonewood Complex Care
Stonewood Complex Care supports people in their own homes across Trafford and the wider Manchester area. We work with older people, people living with dementia, adults with a physical disability, people with visual or hearing impairment and younger adults. A good deal of our work involves situations that standard packages have struggled with. Someone whose health is unstable and whose support has to change week by week. Someone coming out of hospital with a great deal still to sort out. Someone whose circumstances involve several different services that need pulling into one coherent plan rather than left to overlap and contradict each other. In practice that means fairly ordinary things done carefully. We spend longer than most on the first assessment, because getting the detail right at the start saves a great deal of trouble later. We write plans that reflect what the person actually wants their day to look like. We stay in close contact with district nurses, GPs, social workers and anyone else involved, and we chase things rather than assuming somebody else has. We also keep the same carers with the same people wherever rotas allow. Where a situation is complicated, a carer who already knows the household notices a change long before any formal review would, and that is often what prevents a crisis.
