We want to know how the day usually runs, what has become hard, and what the person can still do perfectly well and would rather we left to them. Support planned that way feels like help rather than interference. Continuity is where we put our effort. A familiar carer who knows the house, the routine and the person does more good than any amount of paperwork, and that is especially true where dementia is involved.
