About Home First - South

We work with older people and with younger adults who need help to manage everyday life on their own terms. The name says what we believe. For most people, home is where they would rather be, and the point of care is to make that possible for as long as it sensibly can be. So our starting question is not what somebody cannot do but what they want their day to contain and what stands in the way. Often the answer is quite small: a hand at the start of the morning, a reminder about medication, a hot meal, getting the shopping in, somebody to notice how they are. We plan calls around the person's own routine rather than the convenience of a rota, we keep the team going into a household small and steady, and we give notice when a regular carer is away. Staff are trained before they work alone and supervised in people's homes once they do. We also try to be useful past the edge of the care plan, whether that is noticing a rail would help by the back step or that somebody has gone quieter than usual.
