We are a community care team based in Burton on Trent, supporting people in their own homes across the town and the surrounding part of Staffordshire. Most of our work is with older adults, a good number living with dementia. We also support younger adults, people managing a mental health condition, people with a physical disability, and people who are blind, partially sighted, deaf or hard of hearing. We start from the household as we find it rather than from a list of tasks. Who else lives there and how worn out they are. What the person can still do and would be insulted to have taken over. Which part of the day has actually broken down. From that we agree what we will do, write it in plain language, and change it when circumstances change. Visits run from half an hour to most of a day and cover personal care, medicines, meals, housework, shopping, appointments and company. We aim to send the same few workers to each household, because continuity is most of what makes home care tolerable. We will also say honestly when the support needed has grown past what can be delivered safely at home, because that conversation is kinder had early. We have not yet had a full inspection by the Care Quality Commission, so there is no rating to point you at. Ring us and judge us on the visit.
