About Realise South West
Realise South West supports people in their own homes in Taunton and the surrounding parts of Somerset. We work with adults who have a learning disability, adults who are autistic, and people living with mental health conditions. Our purpose is to make the life someone wants workable. We agree support with the person, in a form they can genuinely take part in, and we review it as circumstances change. Where a person wants to build a skill, our job is to support the practice and then step back, even when doing the task ourselves would be quicker.
