About Connect and Care
We are Connect and Care, a home care agency in Chatham, in the Medway towns. We support people in their own homes, working with older people and younger adults, people living with dementia, adults with learning disabilities and autistic adults, people managing a mental health condition, and people with a physical disability or a sensory impairment. With that spread of needs there is no standard package to offer, and we do not pretend there is. Our first conversation is about the person's ordinary day, what has become difficult and what they want to keep managing themselves, and everything is built from those answers and revisited as circumstances change. Continuity is the part we work hardest at. Sending the same carers to the same households takes more effort to roster and it improves care more than anything else we could do, particularly where somebody finds new people difficult or communicates in a way that takes time to learn. Because the work happens in somebody else's home, how we behave there counts as much as what we do. Asking before we move anything, putting it back where it belongs, and arriving when we said we would. We keep families informed and work alongside GPs, community teams and social workers. Our most recent overall rating from the Care Quality Commission is good.
