Choosing a home care agency in March

Start from the shape of the week, not the hourly rate

Good home care stands or falls on whether the right person arrives at roughly the right time, so begin by mapping what is actually needed: which parts of the day are difficult, how long each call realistically takes, and whether anything is needed at night. The five agencies matching this search in March will each propose a package from that picture. Ask how long their calls are in practice rather than on paper, and whether a fifteen minute visit is ever used for personal care, since that single answer tells you a good deal about how an agency is run.

Continuity is the thing to interrogate

Ask how many different care workers would come in a typical fortnight and who covers sickness and holidays. Ask whether the rota is shared with the household in advance and what the agency does when a call is going to be late. Four of these providers are rated Good by the Care Quality Commission and one is not yet rated, so the ratings will not do much sorting for you. The reports themselves carry the detail, setting out findings on whether a service is safe, effective, caring, responsive and well led, and for home care the safe and responsive sections usually speak directly to missed and late calls.

Questions worth asking at the assessment visit

When an agency comes to assess, ask who will write the care plan and how often it is reviewed. Ask how medication support is recorded, what training staff receive for the specific conditions involved, and how concerns raised by the family are logged and answered. Ask what notice either side must give, and whether the agency can increase support at short notice if someone comes out of hospital needing more help than before.

Arranging support and understanding the cost

The care needs assessment

Anyone who appears to need care and support can request a care needs assessment, and Cambridgeshire County Council holds the duty to carry one out. It is free and is not means tested, a relative may ask for it on someone's behalf, and a GP, district nurse or hospital discharge team can refer. The written outcome records which needs are eligible and is worth having even where a family expects to pay privately, because an agency then starts from a clear account of what is required. A relative providing regular unpaid help can request a carer's assessment at the same time.

Who pays, and how a budget is spent

A financial assessment follows, weighing capital and income against the national thresholds. Where capital falls below the upper limit the council contributes towards the cost of care at home and the person pays an assessed amount; above it the support is funded privately, although the council can still be asked to arrange it. Funded support may be offered as a personal budget taken as a direct payment, which lets a household choose its own agency rather than accept an allocated one. Ask every agency for written hourly rates, including what is charged at weekends and bank holidays, and how travel time and mileage are treated.
