We keep the group of carers around each person small and familiar, we build travelling time into the rounds, and we introduce anyone new properly rather than sending an unfamiliar face without warning. Carers are asked to notice as well as to do, and to raise anything that concerns them with us the same day so that we can involve a relative, a GP or a social worker early.
